Ethical Clearance Documentation

Application form and supporting documentation templates

UQ Business School students seeking ethical clearance should prepare the following documentation (please click on the link to access the document template):

Form E - UQBS Ethical Clearance Application form.

Form E1 - Covering letter.

The covering letter should be addressed to the Chair, UQ Business School Ethical Review Committee.

The covering letter should:

  • introduce the project
  • outline the purpose of the project,
  • clarify the various stages involved, and
  • identify target participants for the various stages of the project.

Form E2 - Participant Consent form plus (Guidelines for Use)

» Combining Forms E2 and E3 into a single Form.

  • It is possible to combine the Participant Consent Form (Form E2) and Participant Information Sheet (Form E3) in many (NOT ALL) cases.
  • For example, if the research involves simply a one-off survey, then the act of reading the Participant Information Sheet and then completing the survey could constitute consent, dispensing with the need for a signed consent form, and also reinforcing the anonymous nature of the survey.
  • If names or identity of participants are not required by the researcher, then a separate Participant Consent Form (Form 2) is not required.

Form E3 - Participant Information Sheet plus (Guidelines for Use)

This will be a sample of the document planned to be circulated to the participants. Information provided will include:

  • The overall approach of the project
  • Purpose and intended outcome
  • Where the research is to be conducted
  • How the research will be conducted (e.g. interviews, survey forms)
  • How long will each survey take/How long will each interview take
  • Whether or not interviews will be recorded

Form E4 - Project Consent form (Gatekeeper's Permission).

This form will be endorsed by the company/group who give permission to the student to conduct the research within their organization/group.
Please note, not all applications may require Form E4.

Form E5 – A copy of the questionnaire to be circulated to participants or a copy of the interview protocol.

Note:

If you decide to combine the information sheet (Form E3) and consent form (Form E2), you should clearly indicate that either a copy will be given to the participant to retain (so they will have contact details of the investigator and the University Ethics Officer) or clearly indicate the 'tear-off' part of the form for the return of the completed consent slip. If the latter method is adopted, the 'tear-off' slip must also include the title of the project.
